Minor students aged 14 to 17 years old will need:

-Original/Certified Certificate of Birth and another primary document or two secondary forms of identification. No photocopies of a birth certificate are accepted.

-Social Security card

-a certificate of enrollment from school (students must be in 8th grade at least)

-Legal Custodial/Domiciliary Parent/Guardian with his or her Driver’s License or State ID Card or Original/Court Certified Custody Papers (if applicable), and two proofs of residence for a parent or guardian.

Adult students who are 18 years or older need:

-Original/Certified Certificate of Birth and another primary document. No photocopies of a birth certificate are accepted.

-Social Security Card

-proof of residence

 

If an adult student is applying for a Real ID, two proofs of residence are required. If an adult has a valid Louisiana State ID already issued and their Social Security Number is in OMV’s system, then only the ID and Proof of Residency is needed.
